Do the workers have to install an app?
No. Tunti runs in the browser on a phone. The worker opens the address, signs in with a PIN and books the hours. It can be added to the phone home screen like an app.

How does the client signature work?

Is there a free alternative for time tracking?
There is, and many start from a spreadsheet or a free time-tracking app. Those are enough when there are a few people and the hours go to one site. The problem arrives when the client questions the number of hours: a free spreadsheet carries no signature and no record of who entered what and when.
How is this different from a spreadsheet timesheet?
A spreadsheet is a writing surface, not a system. It does not chase missing hours, it does not show the site manager where the week stands, and it does not hold up as evidence when a timesheet is disputed.
